Life Transitions Counseling is a faith based counseling practice located in Ponte Vedra Beach, Florida, also serving Jacksonville, Jacksonville beaches, and St. Augustine areas. Owned and directed by Jason Hosch, PhD, LCCC, LMHC, the practice is committed to helping people overcome various hurts, habits and hangups in their lives. These include but are not limited to anxiety issues, addiction, depression, grief, codependency, boundary problems, relationship issues, couples and marriage counseling, anger management, abuse, trauma, and sexual issues. We utilize a holistic approach to counseling, incorporating the needs of body, soul (mind, will, and emotions), and spirit.
